I have what I call daily flares where my pain start's up every day between 4:00-4:30 and just get's worst. I have my alarm clock on my phone set then to start taking my pain med's at specific time's to help fight the pain. Does anyone else have this happen? Does anyone know why this happen's? I hate it.

Yep. Mine usually start around 2 pm, but today started in the late morning. Before Cymbalta it was 8+ pain level unless by some miracle I was able to sleep. I hate it, too. We all do!

Hello Pea,
Imagine it could be one of many things.. 2 I can think of.
- medications wore off as time to take again if taking daily. - eating something each day about same time body is responding to
Is it 4am or 4pm.. if 4am.. then maybe from being asleep for so many hours... I have had that in past.
Hope you can come up with an answer.

I'm pretty sure that mine are the NSAIDs wearing off. I take diclofenac SR twice a day and it should last through 12 hours, but it doesn't. Morning dose at 7.00 but by 2.30 I'm feeling pretty bad. I take tramadol at that point and it gets me through to the next diclofenac at 7.00 pm, which takes me through the evening and till bedtime. I can also get a mini flare up if I don't eat right - I'm usually low starch, but sometimes just get far too much starch and that will have me flaring.

One of the ways I know I am heading into an AS flair is that I start running a low-grade fever between 3:30 & 4:30pm and the pain ramps up at the same time. If I am having a stressful day it may come on even earlier. I also have Lupus and have been told that that is the cause of the fever. When I asked why it seems worse in the afternoon I was told that it is because my body was basically running out of steam and telling me to slow down. He had also suggested a mid day or an early afternoon nap if I need to keep going. This is rarely convenient and I really can’t nap so I have not tested it enough to be able to tell you whether or not it helps. But if you can nap before it starts maybe it will help?

I do try to nap everyday but someday's I can't. It doesn't make a difference pain wise but help's me to be able to get up and cook dinner. My med's are the same and I have my med's on a timer on my phone. I don't let them wear off first. I used to take as needed but I am past that. I am already on the NSD but haven't noticed any change on that. I don't snack in between meal's. I have racked my brain on this to try to determine why the pain is horrible late afternoon up until I take my sleeping med. I am having Rhizotomy's in 4 of my rib's tomorrow. This is where the worst of the pain is. The Percocet and Oxycontin don't even touch this pain. Pea

funny you would mention this, because I have had similar, for three years, every night, I first lay flat on my back, and when I feel settled i roll to my right side and sleep fetal position, but a few minutes on my side and i would not call it pain, it was more like a tickle...strange at it sounds, it would go across half my hip the same pattern and same intensity, and it would only do it one time a night. hasn't done that for a year. but it was old faithful, kinda miss the tickle...

i don't. but do know that when we are sick (with a cold, the flu), we tend to run a higher fever, feel worse, later in the day. feel better

Maybe that is when I sit down finally and have to put my back against a chair. It also hurt's when I drive and sit against the chair.
Had the Rhizotomy's today. I will let you know if they work when the pain wear's off from the procedure.
