these are side effects of opioids but it does seem to me to be a very big jump up in dosage. In particular, the itching is very common and happens to me with even a slight increase. It's very important to increase the dose of opioids slowly and I'm surprised your doc jumped you up like that so rapidly.
If the symptoms continue you need to get checked out. Hubby should watch out for signs of depressed breathing, slow heart etc.
