Survey Introduction and Informed Consent

Independent Study Project
This survey is part of an investigation for a master¹s independent study project. The study is being conducted by Elizabeth Kramer, a second-year master¹s in genetic counseling student at the University of Maryland School of Medicine in Baltimore Maryland. Elizabeth may also be more familiar to you as the Helpline & Resource Assistant at the Genetic Alliance in Washington, DC.
You are being asked to participate in this study because you are involved in a genetic support group. The survey is designed to obtain your views on the relationship between genetic counselors and genetic support groups and your ideas about the functions of genetic counselors and genetic support groups. You will be asked to complete a short, 5-minute questionnaire about the functions of genetic support groups, your involvement with a support group and with genetic counselors, and some demographic information about yourself.
Participation in this project is voluntary; it is your choice whether you fill out and return the survey. Procedures for maintaining confidentiality are as follows. Your name (or any identifying information) will not be connected in any way to the answers you provide to this survey. If you choose to e-mail back the survey, please attach the survey as a Word document so that when the survey is printed out and the e-mail is deleted, your name and e-mail address will not be connected with your survey. The survey is formatted so that you can type in your answers. Alternatively, you may choose to print out the survey, fill it out and mail it back to the address below. Unless you wish to identify yourself, please do not put your name or any other identifying information on the survey.
The results of this survey will be analyzed and the information gathered will be used to identify and describe the current status of the relationship between genetic counselors and genetic support groups. It will also be used to identify factors that play a role in shaping that relationship and to determine whether it would be beneficial to provide education about the relationship between genetic counselors and support groups to genetic counselors and support group members and leaders. The information gathered in this study will be beneficial to you because a strong partnership between professionals and support groups will improve the services both can provide.
To fill out the survey, open the attachment using Word (or another word processing program). Then simply type in your answers on the lines or type an "X" in the boxes provided. For example:

Then, once you have completed the survey, you can send an e-mail to ekramer@geneticalliance.org and send the survey as an attachment (or put it into the body of the e-mail). If you do not wish to fill out the survey on your computer, you may also print it out and fill it in by hand, and then mail it to the address below or fax it to the number below.
Also, if you wish to forward the survey to others who may wish to fill it out, please feel free to do so. If you would like a copy e-mailed to someone else, please send their e-mail address to ekramer@geneticalliance.org.
