In my state (Washington) I was able to download the form off the state website, print it out and take it to my doctor to check the appropriate boxes and sign; then I went in person to the Department of Licensing and got two "permanent" placards (good for 5 years).
Here, it is called the "Disabled Parking Permit", that might help when googling it for your state.
