If you want to use this QR code (Quick Response code) just save the image and paste it where you want. You can even print it and use it that way. Coffee cups, T-Shirts etc would all be good for the QR code.

Before my appt, my doc sent out a packet of forms to fill out. One was a form about pain, locations, what helps, etc.
If you don't get that ahead of the appt, make a list of what hurts, what helps, what makes it worse, etc. Make a list of meds you've taken and if they helped or not. Track your pain levels (and the type of chart you use--Mankowski is my favorite) over time and if your current pain meds help or not. Tell him if you've used TENS, PT, etc, with any benefit.
I've done well with the injections and ablations that my doc has done and recommend you try it if your doc offers and you're comfortable trying it.
You can ask what his/her opinion is of your pain and what are the choices of ways to reduce it.
You could ask how he decides between using injections or using medications for various pain issues. (Likely depends on source of pain, spine vs feet, etc)
You can ask about other methods besides injections and oral meds (narcotics, NSAIDs, etc), like TENS, biofeedback, etc, that he can use if you are a candidate for it.
I would add that you can read any of my accounts of the procedures I've had...most are here on KA. Definitely take a serious look at getting the back injections. They can make a huge difference in the amount of pain you feel. Take the back pain away and your other pain may be more manageable. My feet will never be okay, but when the SI and lumbar pain is gone, it's easier to bear. I love my pain doc. She's a keeper...wish you all could go to her.
Hope your appt goes well...especially after the long wait.
