I think alot depends on your insurance coverage. For mine I dont think I had to have tried any number of meds but they had to submit it and I had to get approval, my approval if for 1 year at a time and before I get the first treatment I have to have a TB test and a chest xray.
I am lucky that I have BCBS and Medicare I pay minimal since BCBS covers most of it then it goes to Medicare and what they dont pay my Dr has agreed to write of the rest, otherwise I could never afford a $6000.00 a month treatment.
