Here is a paragraph from the AS dietary primer:

Persons with RA were almost certainly exposed to the K. pneumoniae the same way in which those with UC, CD, and AS are exposed, but they have no increased production of the antigen over the healthy control groups. This seems to indicate that, in persons who are susceptible to one of the three conditions, a considerable overproduction of IgA follows exposure to the K. pneumoniae antigen, like an allergic response, and the body is adversely affected by the excess IgA; the autoimmune reaction. Those with RA, have increased quantities of IgG to the bacteria p. mirabilis, while those with the three other conditions do not exhibit any increase in IgG production, although certainly these groups are exposed to this antigen, but likely from a different route (2).

Could this sentence ("Those with RA, have increased quantities of IgG to the bacteria p. mirabilis...") mean that the starch free diet would not be as effective for someone with Reactive Arthritis/Reiter's as it is for those with AS ("...overproduction of IgA follows exposure to the K. pneumoniae antigen...")?

Trying to figure out why it's not working...

"In humans, two patterns of cytokine secretion by helper T-cells have been identified. Th1 (but not Th2) cells upregulate production of IL-2, IFN-Gamma and TNF-Beta. Th2 (but not Th1) cells secrete IL-4 and IL-5, but not IFN-Gamma. The Th2 cytokine pattern provides a helper function for B-cell antibody production, particularly of the IgE class. Therefore, it plays an important role in allergic diseases such as atopic dermatitis and asthma."

Dear Sharon:

The three conditions all caused by K. pneumoniae, according to Ebringer are AS, RS, and CD (add iritis to this mix as one of many sub-conditions). Although classic rheumatoid arthritis is not caused by the same pathogen that causes AS et al, there is some evidence that about 60% of RA is related to a UTI (urinary tract infection) from the common 'enterobacteria' P. mirabilis.

As numbers of AS-family provocative pathogens increase, the other bacteria seem to likewise multiply, increasing the risk of re-infection in the kidney-bladder reservoir (or focus of infection), for RA. Campbell was convinced that his dietary regimen worked for RA in addition to AS; the pathogenic relationship is possibly one of the answers, but you are probably correct in many cases of RA not responding to the NSD--but RS should respond as well although there are some complications that can make RS more difficult to treat and in fact it should better respond to colonic cleansings than AS/CD, but there is also a recalcitrant UTI associated with RS, too.

(I never received Ebringer's suggestions for the Primer, but it will be updated soon for clarity...thanks for the input).

I recently came across a paper co-authored by Ebringer reporting a clinical trial which showed that RA patients improved with a vegetarian diet. Bet he didn't like that! He was never complimentary about veggyism. When I asked him what he recommended for RA he suggested drinking plenty of fluid to flush the kidneys and upper urinary tract of proteus.

I'm looking into the colonic cleansing idea as well as fasting. Any suggestions for me? Any suggestions regarding the possible recalcitrant UTI? Feel free to PM me if you'd prefer.

So, then, you don't want to read Braun and Sieper's article, "Cytokines and the Immunopathology of the Spondyloarthropathies," (Curr Rheumatol Rep. 1999 Oct;1(1):67-77) and explain it to me over a cup of helminths? Phooey!
