It is my son's 2nd birthday today and we have the best cake in the world. I want a piece. I have been very successful with NSD but begin to flare with even the smallest amounts of hidden starch. Should I expect a monster flare if I eat something very starchy, and a lot of it? I'm sure ymmv, but what do you all think?
Corey Husband to a fantastic woman, father to two amazing boys. Diagnosed with AS on 10/8/11. Ups and downs with meds after testing the NSD waters in 2011 and, oddly enough, running has led me back to NSD + no added sugars. Feeling great and am pondering my first 50K trail race...

I wouldn't do it, just think of it as poison to you. Remember your worst days and think to yourself- Do I want to feel that pain again? I have never stuck to the nsd this long and for once I am feeling the benefits, although I have a ways to go because I have to wean off of prednisone, which always hurts my muscles, but I'm on th eright track. I don't want you to go thorough a flare up. Think of it this way--- you have been running a race, do you want to fall and stop and have to start all over again, or do you want to keep running and look back at all the progress you made.You can do it!!!! I cannot cheat the least at this point, I don't know if you are like me, but a cupcake gives me great pain.

Diet change has improved my RA. I feel best eating raw veggies and some fruits and avoiding grains, sugars, nightshades, beans and dairy. 34. Some rheumys say AS stage 1-2 some others say USpA Also UC - rectocolitis. UC curently in remission since feb 2011. AS/USpA remission march-aug 2011. Flare - sept-nov 2011 (antibiotics). Remission now... Modified NSD/SCD. Cook your own ! ____________________________________________________________ Mesalazine-Salofalk 500 mg/day And the list of my medication has become verry short after some years on this diet

Well, I did it. Have no real desire to keep eating starch but it is a once-a-year event in my son's bday. I'll report back tomorrow on how I feel.

I think about 3 days of sickness is the norm for me, followed by 3 or 4 days to fully recooperate. The pain isn't the only problem for me.. Also: dry mouth at night, stuffy nose, skin problems, ulcers, and lowered mental capacity in general, ie. Memory, concentration and ability to juggle tasks are all poorer.
I am very easily tempted by situations like the one you have, but after getting sick enough times and the concern that longer term damage may accumulate, ya kinda start to think it isn't worth the five minutes of pleasure.

My biggest problem is that once I partake, I can't seem to stop. Crack Junkie.  Pain, stiffness, swelling after about 4 hours for me. Immediately though: a gurgly belly. Good luck to you! -Donette p.s. I find in these sitatuations that a sugar free jello cup with cool whip on it makes everyone else say "yum, that looks good." It makes them feel more comfortable than having to eat in front of me, ya know?? It's mommy's special dessert.

CHEAT and there is probably a good chance You will flare, but so what? You know what to do to take it down and having a cheat once in a while only supports Your conviction and increases Your resolve.
Just don't get some bad AS complication You have never had before; You are not unfamiliar with the symptoms of inflammation and the world will not end if You cheat and You will not go all the way back to square one but it is a setback that You will have to deal with, one way or another.
I would not make it a habit, unless I have no more reaction to starches (which by now after 14 years I have nearly achieved enough gut healing for diet to be "normal" but always vigilant).
EDIT: OICU812...good luck and I hope You don't flare so easily...

Hope you don't suffer too much. I cheat too, but am only on a low starch diet, so its not always quite so noticeable immediately for me.
I guess I would have cheated somewhat at the party - not to the full extent of having the whole piece of cake, but to having a goodly chunk of the frosting. In that kind of circumstance for me its easier being on a gluten free diet already, but I'd also have made sure there were things that I could have eaten safely that had the celebratory party feel to them. That jelly with coolwhip option sounds good to me, as to meringues (egg white and sugar) and surprisingly quite a few other party options.
Maybe start planning for the second birthday a bit earlier if you are still on the NSD?

Thank you all for your responses/well wishes/advice. It is the next morning and I'd say I feel definitely worse than when hidden starch sneaks through but not as bad as I feared and to celebrate my sons birthday I am not upset at the sacrifice. I know that I am back to strict NSD today and will be feeling better after I exercise and much better by tomorrow.
It certainly IS ENOUGH pain, stiffness and general discomfort to remind me how far I have come and how much I do not want to go back.

Being on NSD and occasional cheating are all part of YOU being in control of your life rather than just falling over and being in total submission to AS. Keep up the good fight!
AS symptoms started 1991. Official dx in 2006 with HLA-B27+, fused SIJ, bone spurs in back, extreme rib/hip pain, and other family with SpA. Started Enbrel in 2006 with good results, but stopped in 2010 due to nerve damage (MS) from it. Getting good results with no-starch diet since 2011.
